Summer Review Appointment Request

Please complete this form as fully as possible and indicate the student’s preferred availability. Library staff will contact you by phone and/or email to arrange a 25-minute appointment within the indicated block of time (morning, afternoon, or evening).

Students are expected to meet with the teacher at the appointed time each day, Monday-Thursday, for the entire 2-week session unless special arrangements are made in advance.

Appointment requests submitted May 29 – June 9 will receive priority scheduling. Requests made after June 9 will be scheduled as space allows.

A parent/guardian must arrive with the student for the first appointment in order to complete their registration.

I have read and understand the following:

  1. Cancellations must be made at least 24 hours in advance. Call Shaker Main Library at 216-991-2030, option 7 to cancel an appointment.
  2. A cancellation less than 24 hours in advance will be considered a "no show."
  3. More than one "no show" will result in the cancellation of future appointments.
