This is the official site for I.R.S. forms. At the bottom of the page is a window you can scroll through to locate any form or instructions you need.

Ohio forms
Follow the link to search for Ohio tax forms. Select “Individual Income Tax” or another tax type from the pull-down menu options. Fill in the appropriate tax year, and select the format you would like to use. (If you are using the Internet computers on the first floor at SHPL Main library, or any of the public computers at the Bertram Woods branch, select PDF (not PDF fill-in) as your document format.)

RITA Page
The Regional Income Tax Agency collects taxes for 110 municipalities/local cities in the state of Ohio. Follow the links listed on the left to find information about RITA and to download and print forms. (If you are using the Internet computers on the first floor at SHPL Main library, or any of the public computers at the Bertram Woods branch, choose the link to access “Blank Forms.” You will not be able to use “Auto Calculate” from these computers.)
